UNIFORMS AND APPEARANCE: Are all employees wearing a slip-resistant shoe? If not, please write their name below and notify the manager on duty. *Employees are NOT allowed to work in the restaurant without the slip-resistant shoes.

CLEANLINESS AND SAFETY: Please Wash, Rinse, and Sanitize (or use the dishwasher) our ice buckets, ice scoops, and ice paddles.

CLEANLINESS AND SAFETY: Is the front counter floor clean (Please take picture of floor)? If not, did you use the Mopit Machine or regular mop to clean? Floors must be maintained cleaned at all times.

CLEANLINESS AND SAFETY: Please check if all LOWBOYS are clean on the inside, FREE of WATER on the BOTTOM. Water on the bottom of the cooler is considered a food safety violation. If not, please write down which one is not clean or have water+clean it.

CLEANLINESS AND SAFETY: Are both Shake base dispenser tubs cut at an angle of 45* degree angle and less than 1 inch? If not, did you take the corrective action?

CLEANLINESS AND SAFETY: Are the scissors that we have in place to cut off the shake base tube completely clean and sanitize after use? If not, did you clean it? (Please add a picture)

CLEANLINESS AND SAFETY: Please walk around the property. Is the parking lot, drive thru area, the grass around our property clean? If not, did you clean it?

FOOD SAFETY AND TEMPERATURES: We should never tell a customer the food ingredients of a product due to allergy. Do all FOH team members, leaders and managers know how to pull the ingredients for a particular entree on the POS? If not, please coach.

FOOD SAFETY AND TEMPERATURES: Record temperature of a salad and a wrap in the lowboy in DT area. If above 40 degrees, please take it to the walk-in cooler and notify manager immediately.

FOOD SAFETY AND TEMPERATURES: Do all TCS items held cold at the front counter have a proper FOOD SAFETY LABEL? (2% milk, prep items, lemons, etc.)

FOOD SAFETY AND TEMPERATURES: Are any labels in the walk-in cooler missing or expired (lettuce, leafs, chicken, lemonades, milkwash, etc)? If yes, please verify the product and correct label, then notify the kitchen manager on duty.

FOOD SAFETY AND TEMPERATURES: Have all condiment utensils been changed out (Lemon tongs, tea spoon, cheery tongs)? They must be changed every 4 hours. If not, please correct it and notify manager on duty.
